Sample Labeling and Transport for HPV/STI/Tropical Fever Assay Specimens

Samples should be packaged appropriately and each sample accompanied by a request/requisition form. Sample labels and requisition forms must match exactly. Samples of different types must be packaged separately.

Minimum Requirements for Label on Sample Collection Device/Container

    • Name/Code of patient
    • National ID number/Passport number
    • Date of sample collection

Request form Minimum Requirements:

    • Name/code of patient
    • Patient Identifier (National ID number/Passport number)
    • Passport number (non-nationals)
    • Date and time collected
    • Source/Requester
    • Test requested

Sample Rejection Criteria

    • Any unlabeled sample.
    • I.D. or Passport # on form and sample do not match.
    • Incorrect sample provided for test to be done.
    • Incorrect sample container that affects test to be done.
    • Leaking/spilled specimen or sample integrity compromised.
    • For blood samples: grossly haemolyzed, lipaemic, icteric or analysis period elapsed.

Sample Transportation

    • Swabs for HPV testing provided by BVL are to be placed in the collection device container, labelled and transported to the lab within 48 hours for regular testing or on the same day for expedited testing. Store swabs at room temperature prior to and after sample collection.
    • Specimens for STI testing should be labelled and transported to the lab within 48 hours for regular testing or on the same day for expedited testing. Store specimens at 2 – 8 °C after sample collection.
    • Blood samples may be stored at 2 – 8 °C for up to 48 hours.
    • Biohazard specimen bags are recommended for use in transporting samples. Requisition forms should be separated from samples to avoid contamination.
